Who We Are
CharterCARE Provider Group RI, LLC (CPGRI) is a 500-member, plus, group of primary care and specialty physicians that has been formed as an Independent Practice Association. It is affiliated with CharterCARE Health Partners, including that health system’s two acute care hospitals: Roger Williams Medical Center and Our Lady of Fatima Hospital.
CPGRI physicians form a critical part of CharterCARE’s innovative Coordinated Regional Care health delivery model. Our doctors and other providers collaborate with our hospitals, cancer center, nursing home, rehabilitation center, urgent care provider and home health services to, with the assistance of sophisticated case management processes, link patients to the most appropriate level of quality care at the right time and place.
CPGRI is governed by a board comprised of prominent local Rhode Island physicians. CPGRI staff leverage the case management tools, resources and experience of Prospect Medical Group to provide timely and individualized guidance to all patients, especially those who are dealing with chronic disease.
