CharterCARE Provider Group RI, LLC (a.k.a. CPGRI) is a powerful association of some of Rhode Island’s most talented primary care and specialty physicians that has been formed to collaborate with CharterCARE Health Partners to create Rhode Island’s first Coordinated Regional Care health delivery model. We are the physician arm of this innovative model and collaborate closely with CharterCARE hospitals, nursing home, cancer center, rehabilitation center urgent care center, and home health services. Working together, we provide patients with seamless access to the right level of care and the right time and place.
