Using MyCARE Info
MyCARE Info is your secure online connection to your Roger Williams Medical Center and/or Our Lady of Fatima Hospital information. It is a fast and convenient tool that allows you to view test results and after-visit summaries.
Setting up your MyCARE Info portal account is simple. Complete a request form with: your name, date of birth, gender and email address.
Your portal account may include sensitive health information including, but not limited to behavioral health, alcohol/drug substance use disorder, communicable diseases, HIV/AIDS, and other diagnostic testing.
With MyCARE Info you can:
- View your health summary
- Access your test results
- View physician reports and discharge instructions
- View allergies and medications
Sign up for a MyCARE Info account:
- Using Google Chrome, go to the chartercare.org
- Click on the “Don’t have an account?” link
- Complete the enrollment form
- Access your email for your one-time user name, password and link to the portal
- Click the link to access the MyCARE Info portal
- Enter your one-time user name and password and click log in
- Follow the directions to enter your new user name and password

Login to MyCARE Info
If you already have a MyCARE Info portal account, using google chrome, go directly to MyCARE Login and select log in.
Enter your user name and password.
If you forgot your user name, click on “forgot login ID”
If you forgot your password, click on “forgot password”

Request Proxy Access
A MyCARE Info portal proxy account can be established allowing secure access to a family member, caregiver, or other representative on behalf of a patient. A spouse, parent, adult child or other patient care representative who is at least 18 years old may become a MyCARE Info proxy if they have been authorized by the patient. Legal guardians must provide documentation of guardianship before access is granted. A proxy account may be established by calling 401-456-2555.
Minor Proxy
Minor proxy access for MyCARE Info allows a parent or guardian to securely communicate on behalf of a child. Legal guardians must provide documentation of guardianship before access is granted.
Adult Proxy
Adult proxy access for MyCARE Info allows you to securely communicate on behalf of a patient who is 18 years of age or older. In order to obtain access, both the patient and proxy must complete and sign the Adult Proxy Access Authorization. Adults 18 years of age or older may access their MyCARE Info portal and invite a caregiver, family member or other patient care representative to share access to their portal account. In order for a proxy to accept an invitation, he/she must also establish a MyCARE Info portal account. The proxy's access is terminated when the patient makes a written or online request to terminate proxy access, or the patient revokes access from within their MyCARE Info account.
