Say hello to CharterCARE Medical Associates.  We are a new, multi-specialty, multi-location physician practice that offers quality primary and specialty care plus the convenience of community-based offices. And we’re looking to be your doctors for life. Our primary care providers are prepared to see you at your convenience. If you need to see a primary care provider simply give us a call and we can make arrangements to see you today. Evening appointments are also available.

Our mission is simple and clear: to provide our patients with exceptional health care services with an absolute commitment to quality and compassion. To us, patients are not billing codes or a patient number…but rather unique individuals with equally unique life circumstances and needs. It’s our job to see beyond the immediate problem and help you design a path to health and wellness.

In addition, as a member of the CharterCARE Health Partners integrated regional provider network, we provide our patients with seamless access to a wide range of physician specialists and outpatient and acute care services at Roger Williams Medical Center and Our Lady of Fatima Hospital, as well as to specialized cancer, elder health, surgical, rehabilitation and home care services.

• The office hours of Victoria Capezza, NP,  DO, Krupali Thakar, MD, and Kristina Burton, NP, at 600 Putnam Pike Greenville are: Monday, 8 a.m. - 4 p.m.; Tuesday, 7:30 a.m. - 5 p.m.; Wednesday, 7:30 a.m. - noon; Thursday, 7:30 a.m. - 5 p.m.; Friday, 7:30 a.m. - 12 noon.
• The office hours of Michael Hayden, DO, Madiha Ali, MD, and Valerie Quebec, NP at 725 Reservoir Ave, Suite 6A Cranston are: Monday, 7 a.m.- 4:30 p.m.; Tuesday, 7 a.m.-4:30 p.m., Wednesday, 7 a.m.-2 p.m., Thursday 7 a.m.-4:30 p.m., and Friday, 7 a.m.-12 noon Phones go off daily for lunch from 11 a.m. - 12 p.m.

• The office hours of Abdul Rahman Al Hout, MD, Mohammad Younes, MD, Ashley Liese, NP, and Dhara Shah, PA at 41 Sanderson Road, Smithfield, are: Monday - Friday: 8 a.m. - 5 p.m.

• 41 Sanderson Rd Suite 206B includes: Dr. Corsi, Dr. Lopez, Dhara Shah; Office Hours: Monday - Thursday 8-4:30; Friday 8-12; Saturday 8-11
