Our Journey to Create a Culture of Safety


CharterCARE Health Partners is committed to being a High-Reliability Organization, which means we continuously work together to improve patient outcomes and provide a safe environment for our patients. We foster a culture of safety in our hospital by following and monitoring our quality and safety practices and continuously seek ways to improve the care provided.
 
Culture of Safety
Safety is our top priority at CharterCARE Health Partners and core to our values. This is why we have committed to becoming a High-Reliability Organization (HRO). In an HRO, everyone from our hospital leadership to our front-line staff is committed to the goal of zero harm to every patient, in every experience, every time. Our culture of safety compels us to hold one another accountable and encourages everyone to speak up about patient care concerns and process improvements and enhancements.
To support our safety culture, we've established behavior expectations for clinical and non-clinical staff. These expectations call everyone to make a personal commitment to safety, be accountable for clear and complete communication, and remove barriers to cross-functional collaboration.

Additionally, we've implemented the following organizational efforts to support our culture of safety and our goal of zero preventable harm:

  • Leaders of all disciplines and departments meet each day for a system safety "huddle" to review all important patient safety considerations.
  • Unit managers and staff employ standardized tools and processes to improve safety practices across CharterCARE.
  • Senior leadership visit clinical areas weekly to reinforce safety behaviors with front-line staff and discuss processes that are working well and opportunities for improvement.

Our hospitals are fully accredited by The Joint Commission, America’s premier monitor of hospital clinical quality and patient safety. In addition, our hospitals have received numerous accreditations for specialized clinical services including stroke care, spine surgery, and hip and knee replacement. We have maintained these reaccreditations year after year by measuring and reporting specific quality and patient outcome metrics.
