Home Health Services are available for anyone age 18 or older. This care is available to anyone throughout Rhode Island and Southeastern Massachusetts who is home bound under the care of a physician and in need of skilled nursing or rehabilitation services. Oftentimes, our patients have been recently discharged from the hospital.

Our Home Health team is made up of registered and licensed practical nurses, licensed physical therapists, occupational therapists, speech therapists, CNAs, social workers and dieticians who provide care to an average of 400 patients each day.

You will remain in (the) comfort of your own home surrounded by your favorite things. We can assist with restoring your daily functions such as cooking your favorite meal, resuming your favorite hobbies or maintaining your home.

Most insurance companies cover the cost, as long as care is provided under the supervision of a registered nurse or physical or speech therapist as part of a treatment plan prescribed by a physician.

Because we are part of a larger health care network, we can truly serve your changing needs and choices. You will have the ability to move from our acute care setting, to our elder friendly nursing facility or any of our outpatient services.

No. Our home health agency accepts referrals from other area hospitals, outpatient clinics, ambulatory surgery centers, emergency departments, doctors’ offices, extended care and rehabilitation facilities, assisted living complexes, patients, and families.

Absolutely. CharterCARE Home Health Services is available 24 hours a day, 365 days a year. Holidays are no exception.

Within 24-48 hours of referral, an experienced professional will visit the person’s home to partner with you and answer any questions you may have regarding your health care plan.
