Rehabilitation Services

The rehabilitation department from CharterCARE Home Health Services employs highly skilled and experienced physical, occupational, and speech therapists to conduct in-home exercise/therapy programs as specified by the patient's physician. Our goal, as always, is to help patients recover safely and independently in the comfort of their own home. To that end, our trained therapists will:

  • Contact the patient to schedule an assessment of physical capabilities and requirements, as well as an evaluation of their home environment.
  • Educate the patient, family, and other health professionals in the therapy treatment and procedures, as well as any equipment usage and home exercise programs.

Physical Therapy

The goal of physical therapy is to restore the patient to his/her maximum physical strength, flexibility, coordination, and independence. Our registered PTs begin by assessing the patient's physical functions and mobility and consulting with the physician to develop personalized treatment plans. Rehabilitative services are available for post-operative joint replacements, musculoskeletal trauma, orthopedic problems, and neurological disorders.

Occupational Therapy

The goal of occupational therapy is to help the patient regain maximum use of his/her upper extremities by improving arm strength, upper body flexibility and hand coordination. Our therapists have experience working with repetitive stress injuries, stroke limitations, and a variety of chronic conditions including arthritis, Alzheimer's disease, and multiple sclerosis.

Speech Therapy

Speech therapy is prescribed to help patients who are experiencing difficulty speaking, communicating their thoughts, or swallowing. Our speech pathologists often treat patients with neurodegenerative diseases, memory and movement disorders, traumatic brain injury and stroke limitations. They begin by assessing the patient's speech, hearing, language, and swallowing abilities and then plan, implement, and evaluate a course of therapy.
